Once a patient referral is made, our dedicated patient care navigators will promptly reach out to the referred patient within two business days. They will continue to provide regular follow-up calls on a weekly basis, until an appointment has been made. Detailed progress notes will be provided after the first session, ensuring that all parties involved have a comprehensive understanding of the patient's journey and any necessary adjustments to their care plan. We are committed to providing exceptional care and maintaining open lines of communication throughout the referral process.
